First Visit

When you arrive for your first visit, please bring with you your completed new patient forms. You can choose to download all of the forms (as a Zipped File ) or you can download them individually below.  If you prefer, you can also call our Naperville office at 630-369-5508 and ask for Rosa.  Rosa, our New Patient Coordinator, will send the new patient packet to your home or office.

What to expect at your intial sleep consultation:

1.  If you have a medical diagnosis of sleep apnea:

Dr. Weyneth and his team will review your current health status, medical symptoms, prior sleep test results and physician notes. They will evaluate your mouth and jaw structures to see if you will be a good candidate for an oral appliance.

Our office manager and sleep team will also meet with you to review your health insurance benefits.

2. If you snore and do not have a medical diagnosis of sleep apnea:

Dr. Weyneth and his team will review your current health status, medical symptoms. They will evaluate your mouth and jaw structures to see if you will be a good candidate for an oral appliance.

Then, we will guide your evaluation process for a diagnosis of snoring and rule out more serious health conditions.
